New Patient Forms/Your First Visit

Welcome to Midwest Dental and thank you for choosing us as your partner in dental health! We would like to make your experience here as convenient as possible. If you have any questions, please feel free to ask any staff member for help.

After you schedule your first appointment, you will receive a New Patient Welcome Packet in the mail with important information to read and fill out.* Below is some additional information regarding your first visit with Midwest Dental.

• What should I bring to my first appointment?
  • Insurance Card
  • Identification – ID, Driver’s License, etc.
  • Patient Health History Form (not for use in Boone, IA, Hastings, MN, or St. Charles, MN)
  • List of current medications: Pre-Medication for Patients
  • Previous dental x-rays: Patient Authorization Form
  • Financial Policy Agreement
  • HIPAA Forms: Notice of Privacy Practices and HIPAA Consent Form
• How early should I arrive to my first appointment?
  • About 15 minutes prior to your scheduled time
• How long should I expect my first appointment to take?
  • 1 hour

*Some situations may call for additional paperwork to be filled out in-office.
