Patient Referral Program

Referrals of your family and friends are always welcome and appreciated at Midwest Dental.  Each day we strive to exceed your expectations and hope to provide the same level of high quality dental care for more patients just like you. Share the exceptional patient care you receive from Midwest Dental. Refer a patient to Midwest Dental and we’ll thank you both with a $20 gift card of your choice to either Target or BP (gas station).

Current Midwest Dental patients must complete a referral card and give to new patient to complete and bring in at the time of their first appointment.  If a patient has not been seen at a Midwest Dental location for 2 years or more they will qualify as a new patient if they return.
