After you schedule your first appointment, you will receive a New Patient Welcome Pack in the mail with important information to read and fill out.* Below is some additional information regarding your first visit with Midwest Dental.

What should I bring to my first appointment?
- Insurance Card
- Identification – ID, Driver’s License, etc.
- List of current medications

- Patient Health History form – yellow sheet that is sent in your New Patients Welcome Pack
